New Patient Forms
To expedite your first visit, we offer online new patient forms. You can fill out these forms at your convenience prior to your appointment. If you prefer to complete them in our office, please arrive a few minutes early to allow time for paperwork.
Being Prepared
To ensure a successful first visit, here are a few things you can do to prepare:
- Talk to your child about the dentist in a positive and reassuring way. Let them know that visiting the dentist is fun and important for keeping their smile healthy
- Gather any relevant medical or dental history information to share with our team
- Arrive a few minutes early to complete any necessary paperwork and allow your child to get comfortable in our office and play in our playroom
